Welcome to the CRESST II Visitor Management System. If you are a NASA/GSFC scientist, interested in sponsoring a visitor through CRESST II, or a visitor that is planning to travel and need support from the CRESST II organization, please fill out the information, as appropriate, below.

SURA Travel Policies Provides definitions and procedural information for various travel related terms and reimbursement expenses. Download Form
SURA Voucher Used to submit travel reimbursement, honoraria and consulting requests for payment. This form should be completed, signed and returned to the Special Programs Manager with appropriate documentation supporting the request. Download Form
IRS Form W-9 This form is used by US citizens receiving compensation in the form of an honoraria, stipend or consulting fee. At the conclusion of the calendar year, a 1099 form will be provided to the vendor for IRS reporting purposes. Download Form
IRS Form 8233 This form is used by nonresident alien individuals, to claim exemption from withholding on compensation in the form of an honoraria, stipend or consulting fee. Download Form
Foreign Wire Request Visitors that do not own a US bank account, or are unable to cash a US check in a foreign country, are able to request that SURA wire their funds to their personal foreign bank account. The wire request must be greater than 50 USD and the visitor must be able to accept USD transfers. Download Form
Rental Car Insurance Letter This letter shall serve as a traveler’s proof of collision and liability insurance when traveling domestically. Travelers shall not purchase the extra collision or liability insurance coverage offered by U.S. rental car agencies for a rental car used for domestic travel.  To ensure coverage is in force, Travelers must note travel is "for SURA" under their signature on the rental car agreement.
